Monthly check-in: a record, not a health score

Use this notebook to keep observations together and bring them to appointments. Monthly is a reminder format, not a safe waiting period for symptoms. Call your vet when something changes. The tool does not diagnose, calculate risk or recommend treatment.

Start with a baseline

Record a date, weight with units in your notes, appetite, drinking, mobility and sleep. Use ordinary descriptions such as 'hesitated before the sofa' rather than a disease label. Record what you do not know. Consistent observations are more useful than a perfect-looking form.
